His diverse background encompasses primary care medicine, chronic disease management, sports medicine, orthopedics, dermatology, surgical wound care, and personal training.

Age 55 and over
Individual: $180/month or $2,000/year
Couple: $350/month or $4,000/year
Age 40 to 54
Individual: $100/month or $1,165/year
Couple: $170/month or $1,800/year
Age 19 to 39
Individual: $95/month or $1,100/year
Couple: $150/month or $1,600/year
Children ages 0 to 18
$50/month - must be enrolled with the same provider with a parent or guardian
One-time enrollment fee
$100 per member
One-time geriatric assessment

Additional Membership Information
Annual discount: Applies when paid in full by check at the beginning of each year.
House calls: Three (3) medically necessary house calls per year are included with membership. House calls are available within a 20 minute radius of our office and subject to provider availability. Additional house calls: $150.
All memberships require a 3-month minimum.
Additional fees apply for discounted labs, prescriptions, and procedures. Costs are to be discussed before applying.
Re-enrollment fee of $250 if patients leave the practice. Does not apply to seasonal members.
